Zia Yoga & Wellness, LLC Liability Waiver

By signing below, I agree that I have read, understand, and agree to the following:

I. The information I have provided in my registration is complete and accurate.

II. I understand and acknowledge that there are inherent risks in participating in yoga and wellness services offered by Zia Yoga & Wellness, LLC. I recognize that participation in yoga and wellness requires physical exertion that can be strenuous. I am fully aware of the risks involved. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in yoga conditioning. I represent and warrant that I am physically, emotionally, and mentally fit and have no medical conditions that would prevent me from participating in the services provided by Zia Yoga & Wellness, LLC.

III. I grant permission to Zia Yoga & Wellness, LLC and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or minors in attendance for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. I hereby release Zia Yoga & Wellness, LLC and its legal representatives for all claims and liability relating to said images or video. Furthermore, I grant permission to use my statements given when providing reviews or feedback, with or without my name, for the purpose of advertising and publicity without restriction. I waive my right to any compensation.

IV. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself and/or minors in attendance (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I and/or minors in attendance may experience or incur in connection with services provided by Zia Yoga & Wellness, LLC.

V. On my behalf, and on behalf of minors in attendance, I hereby release, covenant not to sue, discharge, and hold harmless Zia Yoga & Wellness, LLC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree this release includes any Claims based on the actions, omissions, or negligence of Zia Yoga & Wellness, LLC, its employees, agents, and representatives.

VI. I understand I am responsible for the security of my belongings and should keep any valuables with me at all times or should not bring them into a class or service.
